چکیده

PEARLS & OY-STERS: ICTAL SYNCOPE IN A PATIENT WITH TEMPORAL LOBE EPILEPSY Nitin K. Sethi, New York: Varade et al. reported ictal syncope in a patient with temporal lobe epilepsy (TLE). The MRI revealed a right temporal-occipital cavernous angioma and the seizure showed onset in the right posterior temporal electrode (T6) on EEG, all compatible with a diagnosis of symptomatic localization-related epilepsy. It is unclear why the patient’s seizure semiology suddenly changed and why she started having syncopal episodes while earlier her seizures were characterized by impairment in consciousness and automatisms involving the upper extremities. It is possible that their patient had sick sinus syndrome coexisting with TLE aggravated by oxcarbazepine therapy. A cardiac electrophysiology study would have clarified the diagnosis in this challenging case.

Pearls and Oy-sters: The chapeau de gendarme sign and other localizing gems in frontal lobe epilepsy.

• Unlike temporal lobe epilepsy (TLE), the many semiologic features of frontal lobe epilepsy (FLE) are protean, and often epileptiform discharges or ictal patterns on EEG are not localized. • We highlight 2 lesser-known signs associated with FLE: 1. Ictal pouting, also known as the chapeau de gendarme sign, is an easily recognizable facial expression with its origins in the frontal lobe, seen e...

Ictal and Interictal Electroencephalography of Mesial and Lateral Temporal Lobe Epilepsy; A Comparative Study

Background: Epilepsy is considered as one of the most important disorders in neurology. Temporal lobe epilepsy is a form of epilepsy including two main types of mesial and lateral (neocortex). Objectives: Determination and comparison of electroencephalogram (EEG) pattern in the ictal and interictal phases of mesial and lateral temporal lobe epilepsy. Materials and Methods: This cross-sectiona...

Ictal asystole in temporal lobe epilepsy before and after pacemaker implantation.

Events of ictal bradycardia or asystole may be of relevance in epilepsy patients presenting with ictal falls, and are a potential contributor to SUDEP. The literature on ictal bradycardia or asystole is anecdotal and consists of case reports and small case series. There are no guidelines for the care of patients with ictal arrhythmias. Insertion of cardiac pacemakers may prevent life-threatenin...

Recurrent Syncope Triggered by Temporal Lobe Epilepsy: Ictal Bradycardia Syndrome

Ictal asystole is potentially lethal, and known to originate from the involvement of limbic autonomic regions. Appropriate treatment must include an antiepileptic drug and the implantation of a pacemaker. We report the case of a 54-year-old male with recurrent syncope secondary to ictal asystole triggered by temporal lobe epilepsy. This was confirmed by combined Holter and video-electroencephal...
